Event Covenant Request Form
This plan summary must be completed prior to allocation of resources. If additional hard copy worksheets are required by other agencies, they must be submitted separately. Pay particular attention to lead times for actions related to volunteers.

Event Metrics
Use this form to report metrics from your event if no funding was requested. Please submit this information as soon as possible after your event.

Event Results & Summary Form
Use this form for reporting if funding from Embrace Baltimore was received. Please submit this form within (1) week of your event. Please also remember to return all unused funds to Embrace Baltimore.
